The claimant must submit a claim within 6 (six) months of the claim event date to the administrator by phone call or email using the details below:

*All claim documentation must be submitted to the Insurer within 12 (twelve) months of the date of the claim event. Failure to do so could result in the benefit being forfeited, unless there are extenuating circumstances for the late submission.

Your claim document checklist

Please provide us with the following documentation to enable us to process your claim as soon as possible.

A fully completed claim form signed by the principal insured /beneficiary.
A certified copy of the death certificate.
A certified copy of the DHA 1663 – Notice of Death Form.
A certified copy of the deceased’s Identity Document.
A certified copy of the beneficiary’s Identity Document.
Proof of account of the beneficiary not older than 3 (three) months.
Proof of address of the beneficiary not older than 3 (three) months.
A police report for claim events related to motor vehicle accidents, suicide, murder, and causes of death that are under police investigation.

There are no waiting periods on accidental death, provided the first premium has been paid. In the event of natural death, the waiting period will be 6 months from the policy entry date. Should suicide be the cause of death, a waiting period of 12 months from entry date will apply.

On the Legacy Gold plan, you can cover yourself, your spouse & 6 children. The Legacy Silver plan offers cover for yourself.

The insurer will not change or vary the premium rate during the first 12 (twelve) months after the commencement date of the policy, unless there are reasonable actuarial grounds to change or vary the premium rate or when the variation will be to the benefit of the principal insured. After the first 12 (twelve) months, the Insurer reserves the right to review and change the premium rate and cover annually.

Any changes to the premium rate will be notified to the principal insured 31 (thirty-one) days prior to the change taking effect.  It is important that you take into account your own needs and the needs of your family before taking out this policy. Should your circumstances change, please contact us on 010 001 1001 from 08:00 to 16:00 Monday to Friday, excluding public holidays.

If you miss two consecutive payments, your policy lapses and no funeral payment will be made. You will receive an SMS with account details for payment and you can call us on 010 001 1001. There is a grace period of 31 days following a missed payment for you to make payment.

We doubt you will be, but should you wish to cancel, this policy carries a 31 day cooling-off period in which the policy can be cancelled, should you not be satisfied with the policy or with the service received. Any premium deducted during this period will be refunded should the policy be cancelled provided no claim event has arisen or any benefit has been paid.
